How to Access the Questionnaire Module

1. Navigate to the Questionnaires tab → click on Create Questionnaire.

   

a. For those accessing from the Desktop view → Select the Create Questionnaire icon below.

b. Alternatively, you can access it from the Dashboard view → click on the title Questionnaires.

How to Create a Questionnaire

1. Once you are on the Create Questionnaire page, you will need to select the Fleet and the specific Vessel.

   1. Vessel name: select the dropdown for vessel and loadline (if multiple).

   2. Questionnaire: select the questionnaire type(s) either by typing in the yellow text box or expanding the dropdown → then click on the Create button.

      

c. You can also search the database to find a questionnaire by clicking on the Search button:

Q88 Onboard Users: Please note that you will only be able to view the vessels you have been granted access to.

3. Complete the questionnaire with the missing information or update an existing one.

   

Note: There is a legend at the bottom of the page with an explanation for each icon:

To select the next action click on the Click to Continue button.

Note: Questionnaires must be created in the Q88 Dry platform, they cannot be uploaded.
