If you require a copy of any available questionnaires in the system, you can find it under Questionnaires → Blank Questionnaires.
The questionnaires are saved alphabetically and you can use the search bar/dropdown list to search for specific questionnaires:
After clicking on a questionnaire, a new window will open and the questionnaire will be attached to the email you receive.
