Blank Questionnaires

If you need to get a copy of any available questionnaires in the system, you can find it under Questionnaires--> Blank Questionnaires.

They are saved alphabetically or you can search on the text box:

You click on the required questionnaire, a new window opens and the questionnaire will be attached to the email you receive.
