How to download a questionnaire
After creating a questionnaire, go to the bottom of the page and find the Click to Continue button.
Download a pdf copy of the completed questionnaire
You select the Download to pdf.
A pdf file with the information as completed in the Questionnaire will be downloaded to your computer.
